No, you don't lose marks if you go off-topic. You can add information you like in your answers, but it should be relevant to the topic.

Suppose you get a topic 'education' & you don't know what to speak. You can go off-topic by discussing school, college, institutes, etc. in your city.

But, if you completely avoid talking about education, it's going off-topic. So, if you never mention the main topic, the examiner will mark you low.
